You will need the following CRM codes to create your form:
If you do not have these codes, contact the ITS CRM Team at dl-crm-team@apps.maricopa.edu to get them before continuing. |
Go to Structure > Blocks > Add CRM Form Block Block layout > Custom block library
Click + Add custom block
Click CRM Form block
Add a Block Title if desired. This will display on the page above the formdescription
Add the CRM Form ID provided by the CRM Team (see above)
Add the Form Title to display at the top of the form
Select your college as the Default College. This pre-fills the Preferred College drop-down with your college
Add the Campaign Code if desired (see above)
Add the Term if desired. This is a marketing value that can be used to filter leads in CRM
Add a Thank You Message. If one is not entered, the following will be used:

Add a Redirect if desired. If a URL is entered (external or internal), this is where the user will be redirected to after submitting the form. If nothing is entered into the Redirect field, the user will remain on the same page with the contents of the Thank You Message field displayed in place of the from.Under Region Settings, set the default theme drop-down to

Placing a CRM Form Block
Go to Structure > Block layout
Click the Place block button for the region you wish the CRM Form Block to appear on the page. We recommend placing the block on a page in one of the content regions. Sidebars will work, but it is not recommended.
Update the Visibility Settings as described in Using Blocks Click the Place block button next to the Block description you entered in the above steps
If necessary, update the Title of the block to find it easier on the Block layout page
Uncheck the Display title checkbox
Update the Visibility settings to get the block to appear on desired pages
Click Save block
Review your site to make sure your new block is appearing only on the intended pages.
