The RFI Form Block is a form that can be placed on a page to collect basic information from prospective students. This information gets fed into MCCCD’s CRM system. You can use this form throughout your site as needed. You cannot modify the fields on the form.

Instructions

Creating a RFI Form Block

You will need the following RFI codes to create your form:

  • UTM Campaign Code -  this is the unique identifier for a specific RFI marketing campaign.

If you do not have these codes, contact the ITS CRM Team at dl-crm-team@apps.maricopa.edu to get them before continuing.

  • Go to Structure > Block layout

  • Add a Block Title if desired. This will display on the page above the form

  • Add the Form Title to display at the top of the form

  • Add the UTM Campaign Code (see above)

  • Update the Visibility Settings as described in Using Blocks

  • Click Save block

  • Review your site to make sure your new block is appearing only on the intended pages.

Updating or Deleting a RFI Form Block
